On January 28, 1986, at 11:38 a.m. EST, NASA lifts off the space shuttle Challenger. Seconds later in flight, the shuttle broke up. The investigation found that the disaster was caused by the failure of an “O-ring” seal in one of the two solid-fuel rockets. There were no survivors of the seven-member crew.
